In order to come to the studio in person, this form must be completed and submitted to Dr. Kuhny by the morning of your lesson.  If yes is answered to any question, you can still have an online lesson or the lesson will be credited to your account.  The health and safety of students, families, and staff is very important.

All answers will be kept confidential.  If you would prefer not to answer and know you need to cancel or have an online lesson for any reason, including exposure or illness, feel free to contact Dr. Kuhny directly.
